See attached fuel surcharge schedule for payments made on loaded miles.

PLATE PROGRAM

1st year 100% chargeback to owner operator.  25% reduction annually.

DETENTION at delivery

$30 per hour after two hours at receiver. This must be documented on the bill of ladings with an arrival time and a departure (empty) time. In addition, dispatch must be notified that you are being detained prior to the two hours window expiring.

HANDLING $25 per hour to assist in loading or unloading. This must be documented on bill of ladings and preauthorized by dispatch.

NEW YORK CITY PAY - $175.00 per trip to NYC Boroughs

Full Time Status:

Full-time status is given to employees who regularly work over 30 hours per week or 1800 miles per week over a 3 month average

PLACARDED LOADS

WHEN TAKEN FROM PICKUP TO DESTINATION - $5

STOPS – $40 FOR ALL STOPS OTHER THAN ORIGIN AND DESTINATION TO PARTIALLY LOAD AND/OR UNLOAD FREIGHT

CITY PAY $19.75 per hour when hired to make deliveries and pickups for loads other than your own

 
 

OTHER:

· FED EX or UPS on Styer Accounts will be $25 per use
· Payments to 3rd parties on behalf of employees will cost $1 per payment
· Cell phones are expected as means of communicating with Styer. The prior communication contribution is built into the base mileage raise.


HEALTH/DENTAL INSURANCE

Health and Dental plans are offered to full time employees.  Employee's pay no premium for themselves.  There is a charge for dependent coverage. 
 
LIFE INSURANCE

A $10,000 life insurance policy will be provided by Styer Transportation. The plan will be offered after 90 days of employment to workers maintaining full time status.

RETIREMENT PLAN

A 401K plan is available to employees averaging over 30 hours per week. One year of service is required before entry into the plan. Styer contributes 100% of the first 3% of employee contributions and 50% match on the next 2% of employee contributions.

PERSONAL TIME POLICY:

Pay is based on prior calendar year 52 week average.

After 1 year - 5 days

After 3 years - 10 days

After 7 years - 15 days

After 12 year – 20 days

Sustained full time status is required to work towards receiving personal time benefits. The date used for calculating personal time is the anniversary date of employment. Personal time is not an accrued benefit and will not be paid upon termination of employment regardless of reason.

Because Styer is a small company, we rely on everyone to be available to work during our regular business days. We do understand, however, that illness to employees and family members occur. There are no specific number of sick days allowed. The days are expected to be taken only when necessary. If it appears that this policy is being abused, a discussion will take place and limits may be placed on these days. If you are away from work for three consecutive days, however, a written notice from a doctor is required.

Requests for personal time will be discussed and evaluated for approval.

DEATH IN THE FAMILY

Time will be allowed to attend funerals and make arrangements if a family member dies. In the case of the death of a spouse or children one week will be provided. Three days will be provided for the death of a parent, sibling or grandparent.
